What Is CO₂ Laser Resurfacing?

CO₂ laser resurfacing uses carbon dioxide laser energy to create controlled areas of ablation and thermal injury within the skin.

At View Laser, Dr. Ray uses the SmartXide DOT® fractional CO₂ laser. Fractional technology treats microscopic columns of skin while leaving surrounding areas untreated, allowing the skin to undergo a controlled healing and remodeling response. SmartXide DOT has been clinically studied for concerns including wrinkles, photoaging and scars.

As the skin repairs itself, collagen remodeling occurs within the treated tissue. This process develops over time, which is why the changes following CO₂ resurfacing continue beyond the initial healing period.

What Can CO₂ Laser Resurfacing Treat?

Depending on the patient and treatment settings, fractional CO₂ resurfacing may be considered for concerns including:

- Fine lines and wrinkles

- Crepey or rough skin texture

- Sun-damaged and photoaged skin

- Certain brown spots and uneven pigmentation

- Atrophic acne scars

- Other selected scars

- Uneven skin texture

- Overall skin quality

CO₂ is particularly well established as an option for atrophic acne scarring, although no single treatment is best for every scar or patient. Systematic reviews support fractional CO₂ as an effective treatment option, while also showing that results, recovery and risks vary according to the technology, settings, scar characteristics and patient. Melasma requires special consideration and is not treated in the same way as ordinary sun-related pigmentation.

In addition, CO₂ treatments have been clinically shown to prevent and reduce instances, progression, and recurrence of non-melanoma skin cancers.

How Does CO₂ Laser Resurfacing Stimulate Collagen?

The interesting part of CO₂ resurfacing is that the treatment you see on the surface is only part of the process.

The laser creates precise microscopic treatment zones that remove targeted tissue and produce controlled thermal effects in the surrounding skin. That controlled injury initiates the body's wound-healing response.

During the weeks and months that follow, the skin undergoes repair and collagen remodeling, which contributes to gradual changes in texture and the appearance of scars and wrinkles. Histologic and clinical research supports collagen reorganization following fractional CO₂ treatment.

In other words, the laser treatment happens in one appointment, but the remodeling process continues long afterward.

Why Do CO₂ Laser Settings Matter?

This is one of the most important things Dr. Ray wants patients to understand about CO₂ resurfacing — CO₂ is not a one-setting-fits-all treatment.

Treatment parameters can be adjusted according to the patient's skin and the concern being addressed. Dr. Ray considers factors such as:

- Skin type and pigmentation

- Skin thickness and overall skin health

- Treatment area

- Type and depth of scarring

- Degree of photoaging

- Desired level of correction

- Previous treatments

- Acceptable recovery time

Energy, pulse characteristics, density and treatment pattern all influence how the laser interacts with the skin. For example, treating delicate skin around the eyes is very different from treating acne scarring on the cheeks. Treatment also needs to transition appropriately between treated and untreated areas to create a natural-looking result. The device matters. The way it is used matters just as much.

What Is Recovery Like After CO₂ Laser Resurfacing?

Recovery depends on the treatment intensity and the individual patient. Following treatment, the skin can initially appear raw, red and swollen. Crusting or flaking can develop during the early healing period, followed by residual pinkness or redness that gradually improves.

Patients undergoing a more intensive treatment should expect a different recovery than someone receiving a lighter resurfacing treatment. This is why Dr. Ray discusses both the desired result and the recovery commitment when developing a treatment plan. Temporary redness, swelling and pigmentary changes are recognized potential effects of fractional CO₂ resurfacing. Post-inflammatory hyperpigmentation is an especially important consideration in treatment planning for patients with greater baseline skin pigmentation.

CO₂ is an ablative laser treatment capable of creating significant controlled injury in the skin. This means appropriate patient selection, treatment parameters and aftercare are important. Potential complications can include prolonged redness, changes in pigmentation, infection and scarring.

His approach begins with assessing whether CO₂ is appropriate in the first place, followed by selecting treatment parameters based on the individual patient's skin and goals. When looking at CO₂ before-and-after photographs, Dr. Ray also encourages patients to look beyond the dramatic transformation. Pay attention to transitions around areas such as the eyes, jawline and hairline and how naturally treated and untreated skin blend together. Sometimes the details tell you as much about a treatment as the overall result.

Why Does Skin Preparation Matter Before CO₂?

Dr. Ray views CO₂ resurfacing as part of a comprehensive skin plan, rather than an isolated procedure. Healthy, appropriately prepared skin provides a better foundation for an intensive resurfacing treatment. Depending on the patient, a treatment plan may include medical-grade skincare and other procedures before or after CO₂.

Dr. Ray may recommend treatments such as RF microneedling, ultrasound-based treatments such as Sofwave or platelet-rich plasma (PRP) at different stages of a patient's plan when appropriate. Not every patient needs the same preparation. The goal is to assess the skin you are starting with and build a treatment plan around it.

Why Does View Laser Combine CO₂ With PRP?

Platelet-rich plasma, or PRP, is prepared from a patient's own blood. The blood is processed to concentrate platelets, which contain naturally occurring growth factors involved in tissue repair. PRP can be incorporated into selected CO₂ treatment plans to support the healing environment following resurfacing.

There is clinical evidence behind this combination. Systematic reviews examining fractional CO₂ plus PRP for acne scars have found improvements in clinical outcomes and patient satisfaction compared with fractional CO₂ alone. Some studies have also reported shorter crusting periods and reductions in post-treatment redness and swelling, although protocols vary between studies.

At View Laser, PRP may be applied or used as part of a broader regenerative treatment plan depending on the patient's needs. This doesn't mean PRP is required with every CO₂ treatment. It is another tool Dr. Ray can consider when developing an individualized plan. However, he strongly recommends pairing CO₂ with either PRP, commercially prepared human growth factors or exosomes in order to significantly reduce healing time and improve outcome. This ensures the comprehensive treatment doesn’t just injure the tissue but also provides the nutrients that regenerate healthy tissue.

How Long Does It Take to See CO₂ Laser Results?

The first stage is healing. The next stage is remodeling. Changes in skin texture and quality can begin to become noticeable as the initial recovery resolves, but collagen remodeling continues for months following treatment. This is an important distinction because a photograph taken shortly after the skin has healed does not necessarily represent the final outcome. Dr. Ray encourages patients to think of significant skin rejuvenation as a process rather than a quick fix. The treatment happens in a day. The rebuilding continues for months.

Who Is a Good Candidate for CO₂ Laser Resurfacing?

CO₂ resurfacing is not appropriate for everyone. Skin type and pigmentation, medical history, medications, active skin conditions, history of scarring, previous procedures and the concern being treated all need consideration. A consultation allows Dr. Ray to determine whether CO₂ is appropriate or whether another technology may provide a better balance of potential improvement, risk and recovery for your skin.
